By completing and submitting this Faculty and Preceptor interest form to the proposed Baptist Health Sciences University College of Osteopathic Medicine (BUCOM), you acknowledge that you are interested in a position as a faculty member or preceptor and further acknowledge that your name and the information you provided may be submitted to the Commission on Osteopathic College Accreditation as an example of persons interested in faculty appointments at our institution and if appointed, we may also publish your name and appointment in our directory of faculty members. In addition, the completion and submission of this form is neither an offer nor a guarantee of employment with the Proposed Baptist Health Sciences Univerity College of Osteopathic Medicine. If a faculty appointment is granted, it is not an employment offer nor does it constitute an employment contract or any offer of employment, expressed or implied, and shall not be deemed by any person, including BUCOM or any preceptor or faculty member, to create any legally binding contract between such individual and BUCOM.
